San Diego Family Care (SDFC) is a non-profit community health center established in 1972 for the purpose of delivering the highest quality medical and mental health care services to families in San Diego County.

SDFC clinics bill for their services to the extent that low-income families are eligible for insurance. However, since so many families pay for services on a sliding fee scale, or are uninsured and do not pay at all, the clinics rely upon contributions from the private sector to fill the ongoing gap between revenues and expenses.

SDFC gratefully acknowledges individual and corporate contributors who support the critical importance of its work in maintaining the public health of San Diego County. Examples of programs urgently needing support at this time follow:

1. MEDS-FOR-KIDS PROGRAM : Five dollars will provide one prescription to one sick, uninsured child. One thousand dollars will enable 200 prescriptions to low-income children.

2. ADOPT-A-MOM PROGRAM: Five hundred dollars will provide nine months care to an uninsured, pregnant woman (or teen).

3. ADOLESCENT HEALTH PROMOTION PROGRAM : Pregnancy prevention is taught as well as education and awareness in our Teen Clinic.

4. LABORATORY TEST FUND : Provides funds for lab tests for uninsured patients who only pay for the visit to the doctor, but cannot pay for their lab tests.

5. MEDICAL EQUIPMENT FUND : SDFC clinics deliver 92,000 medical visits per year. State of the art medical equipment is critically needed at all times to insure the delivery of the highest possible quality of care to the community. Revenues from patients and grants are never sufficient to purchase new medical equipment or replace obsolete equipment. Your donations will insure our success in maintaining a high level of public health in San Diego.

6. INFORMATION TECHNOLOGY FUND : San Diego Family Care urgently needs funds to purchase and maintain state of the art hardware and software. This will allow us to capture revenues from third party payors, maintain our cash flow, determine financial eligibility of patients and deliver high quality care. San Diego Family care is advancing to the use of electronic medical records. Donations to facilitate this project are needed.

7. DIABETES SCHOLARSHIP FUND : For a contribution of $750, months of diabetes care will be provided for one uninsured individual. Several individuals can be sponsered. This is truly an urgent need in the communities we serve.

ALL DONATIONS MAY BE MADE PAYABLE TO: San Diego Family Care and mailed to.
